Bug: some form control labels don't access the control, and some radio buttons are not grouped correctly

Though this mainly applies to the 2.3.0 patches...several of the control labels aren't connected to their control using the 'for' since the control may be lacking an 'id' Also, the radio buttons don't seem to have an 'id', and their names are based on the id, HOWEVER, to be grouped, they must used the same name.
