Bug: Form module doesn't correctly initialize from a clean install in 2.1.0

This problem only seems to occur from a clean install of 2.1.0 as other version don't exhibit it and if the form module was already working, an upgrade does not affect it.
If you create a new form module (first in the system), the form settings, edit form, and edit report, links all go to a page not found message. These links refer to a form id# of 0 which is impossible to work with since auto-increment begins as 1.
