The console warning is expected and has nothing to do with ckeditor (we simply need to parse the file via php, but use it as javascript).
I'm unable to reproduce the icon anomaly (though I've seen it in the past). It likely has something to do with the skin packaging since 'kama' the old v3 skin is NO longer the default and the ckeditor package building leaves a lot to be desired.
I'll try to keep working at making sure or testing whether or not the kama skin has the correct file (minified/compressed icon file in the correct sequence), but this is all up to the guys at ckeditor.com)

This problem is the result of chrome being very stubborn on fully refreshing its cache and it hold an older copy (v3) of the icon.png file needed for the toolbar.
Clearing the browser cache should eliminate this issue. But again, chrome is pretty stubbon about leaning on its cache and keeping hold of it (for speed)
