Standardize display/location of add new item links

Most likely this should be at the top of the module where the majority of templates display the "add" link. However, some views still place it at the bottom of the module such as the filedownloadController.
Should probably also standardize the "icon" used for adding to the green circle plus

We should have them all at the top, and with each iteration of a record item, will place a link below the item so that adding a record after that item is possible.
