* Feature request - Add module location_data to form data record

We probably should add the module's location_data to tag a form data record with where it came from. Could come in handy if say you have a single (standard format) contact form, but it's placed on many different pages with those module settings being different...e.g., a contact form for each branch/department that goes to that dept head, but all are stored in one central database.
