Bug: Title may lose/modify embeded quotes

There are reports that (single?) quotes are sometimes dropped from module/item titles or are displayed with a slash (escaped). This may or may not be a new bug associated with our 'scrubbing' all input...as all entries are 'escaped' prior to being saved in the database.
